You'll have to pay $5 for this, but you can obtain a business entity status report online which will tell you the private school's agent for service of process (the person and/or address to whom notices of lawsuits must be served on). The link is below. I'm not certain about this, but it looks like you can request the same information by mail and not have to pay the $5.00. Good luck.

The summons would most likely be directed to the person who signed the contract on behalf of the school. You may find some more info here - civiltree or use this service to issue the summons etc.

You would have to find information on the Board of Directors/Trustees that is in charge of the school in question and get the name of the Chairman/woman. Have you even tried bringing it up to them? Because you may be able to resolve it without even taking any legal actions.

This is a question your lawyer should be able to answer. If you don't have a lawyer, I'd strongly suggest you get one.
